What kind of couplers are you using? Are these rigging couplers like for a
pull-pull setup? Or are they solder on types for pushrod ends?

If they are solder on type  going on pushrods then I suggest using the
Central Hobby CF pushrods and they have 3mm couplers.

For future Ref #4 screws (4-40) are 0.113" dia and the 3mm dia is 0.118"
also shown on Central's Website this gives the proper sizes for the US
thread sizes

I have used the 2mm MK clevis and the 2.5mm ones on 4-40. I usually run a
Tap into them before use. When using the 2mm ones you need to be careful as
the holes are a little small....If not careful you can damage the clevis.
