I was curious about this procedure last winter and did some research.
The form is obtained from the AMA and is filled out in duplicate.
The requester states his reason for asking to move back.
He then has a CD and his AMA district contest board member sign it.
You can find the contest board member list in the back of every other month's Model Aviation or on the AMA website.
After that it goes to your AMA district VP who forwards both forms on to Muncie after he signs them.
AMA Muncie does not approve, disapprove, grant or deny these request. They only place one copy on file
and return the other copy to the requester.
Done.
As a CD I would sign anyone's request that was sincere for whatever the reason.
I bet the contest board member would be the same.
The AMA DVP will sign based on the previous recommendations.

Under current rules you can move up but not down. However, there is a very specific exception that allows one to petition the AMA by submitting a form that, once approved, allows reclassification to a lower class due to various reasons including, but not limited to, disability, breaks from flying, etc...
