Tom, the most efficient method for me has been to call a buddy with a laser
and have him bust them out for me. There are several I have worked with, but
prices have gone up so much as of late, I now do most prototyping by hand
again. 
 
As for fuses, they are pretty crude for a reason. And the big watchout is
that there are good profiles out there, and then there is mass marketed
garbage. 
 
I'll shoot you some stuff off list that I think you'll enjoy : )
